Gotchas

The listing form does not exist until the badge check passes. Step 3 is unreachable, so there is no way to see what will be asked for before the badge is live on your site.
Their snippet asks for a do-follow link to your listing URL, which is stricter than the usual badge and means a site that nofollows its footer links has to make an exception.
